Welcome to Operation Blood Draw Phlebotomy (“we,” “our,” or “us”). By using our website, scheduling an appointment, or receiving services, you agree to the following Terms of Service. Please read them carefully.
1. Services Provided
Operation Blood Draw Phlebotomy provides mobile phlebotomy services, including but not limited to:
Specimen collection based on a licensed provider’s lab order.
Transport of specimens to authorized laboratories (Quest, LabCorp, ExamOne, etc.).
Other services as outlined at the time of scheduling.
We do not interpret lab results, provide diagnoses, or replace the care of a licensed medical provider.
2. Eligibility
You must be 18 years or older to schedule services for yourself.
A parent or legal guardian must authorize services for minors.
A valid physician’s lab requisition form is required for all specimen collections.
3. Payments & Fees
Services are self-pay only unless otherwise noted.
Payment is due at the time of service.
Fees are published on our website and may vary by service type, location, or urgency (e.g., STAT draws).
Travel fees may apply outside designated service areas.
4. Cancellations & No-Shows
Appointments must be canceled at least 24 hours in advance.
No-shows or late cancellations may result in a fee.
5. Privacy & Compliance
We comply with all applicable laws, including HIPAA, to protect your personal and health information.

6. Limitation of Liability
Operation Blood Draw Phlebotomy is not responsible for delays, errors, or issues caused by laboratories processing specimens.
We are not liable for indirect, incidental, or consequential damages arising from the use of our services.
In rare cases of specimen collection complications (e.g., bruising, fainting), we are not liable beyond the cost of the service.
7. Refusal of Service
We reserve the right to refuse or discontinue services if:
A valid lab requisition form is not provided.
Unsafe conditions exist at the service location.
A client behaves in a threatening or inappropriate manner.
8. Updates to Terms
We may update these Terms from time to time. Updates will be posted on this page with a revised effective date.
